2016 New England Supply Chain Conference and Exhibition - Registration Details

REGISTRATION DETAILS

ONLINE REGISTRATION:

Register using our Secure Online Registration Process (credit card only - use the register by "Mail" form/link below for all payments by check). NOTE: This is a new Online registration system = if you have ANY issues/questions, please call direct to ISM Customer Service @ 800-888-6276, option 8, and a representative will walk you through the registration process (or they'd happily take your registration over the phone as well). Also, we created a quick instruction sheet, if you get stuck, and don't want to call ISM = here is the link to the instruction sheet.

Note #1: NEW ACCOUNT LOG-IN REQUIREMENTS
If you are not a member of the Institute for Supply Management (ISM), you will have to CREATE a new account/profile, to register online, (i.e., provide only basic event registration information). NOTE: This one-time account creation will simply allow you to register for the conference, and does not sign you up to be a member of ISM (there is no cost to do this, and ISM will not solicit you afterwards, without your consent, nor add you to its member database). Just click on the appropriate link on the login page and follow the instructions. We will use the information you provide to create your name badge for the conference.

Note #2: DISCOUNT CODES/PRICE
If you are registering through the Early-Bird discount date of Sept 16th, you WILL get the Early-Bird Discount rate. It will show the full (non-discounted) rate until you get to the very last Checkout page. On this last Checkout page, please enter in the appropriate Discount/Coupon code (from the below), for the group that you belong to, and you will then see the discounted rate show up:
     • APICS-NS = APICS North Shore
     • APICS-B = APICS Boston
     • NSMG-ISM = Northeast Supply Mgt. Group / ISM
     • NERT-CSCMP = New England Roundtable
     • OTHER = All other groups, or not a member of any group (you will still get the discount if using the "OTHER" group code – we just need to know where the registrations are coming from!!).

ALTERNATIVE REGISTRATION INSTRUCTIONS:

• PHONE: Preferred method: Call 800-888-6276 (Monday-Thursday 10:30 AM – 7:30 PM EDT, and Fridays from 10:30 AM – 6:30 PM EDT), or
• EMAIL: By using this link to print out the form, and email a scanned copy to Emily Ryan at: eryan@instituteforsupplymanagement.org (to  keep things secure with the email registration, please do NOT include credit card info; ISM's customer service representative will call for the information), or
• FAX: By using this link to print out the form, and then fax it to 480-752-7890, (credit card info is o.k. to fax). NOTE: Fax registrations may take up to a week for us to get you a confirmation back (via telecon), or
• MAIL: By Mail (snail mail address is at the bottom of the form) – OK to include credit card info with the mailing option.  NOTE: Mail registrations may take up to a week for us to get you a confirmation back (via telecon).

NOTE: REGISTRATION DEADLINES
The online registration is available only through September 28th. After that date, you will have to print the registration form (from this link), and fax the completed form (including payment/credit card information) to 480-752-7890 (Attn: Emily Ryan) no later than Thursday, September 29th @ 3 PM. After that time period, you will need to do a walk-in registration on Monday morning October 3rd, at the conference. So be sure to register soon!

*GROUP DISCOUNTS: If registering 8 or more attendees from the same company, deduct $10 per registration fee.  Group registrations must be made by getting preapproval of our Registration Chair at registration@newenglandsupplychainconference.org.  All members of the group must be registered at the same time. This group discount entitles your company to receive a "Corporate Supporter" status, which includes (in addition to the discounted rates), having your company logo and name on a significant amount of our promotional materials (including website, brochures, etc). To have your logo included on the NESCON web site, please have the person in charge of your registrations send the logo to programs@newenglandsupplychainconference.org once your registration process is complete.

**STUDENT DISCOUNTS: Full-time students (full-time is defined as a minimum of 12 credits for undergraduates and 6 credits for graduates) with proof of matriculation at the time of registration. Very limited number of discounted Student Registrations, so register quickly!
NOTE: Student discounts must be pre-approved via a request to registration@newenglandsupplychainconference.org, and cannot be entered via the on-line registration process.
A) Please PRINT (first) and manually fill out the mail-in form (with no payment/credit card information yet), and
B) Scan and send in the filled-in form as an e-mail attachment , when requesting your student discount (along with a scanned copy of your proof of matriculation).
C) Upon approval, you will then be instructed where to mail in the pre-approved form with the discounted payment.

***UNEMPLOYED DISCOUNTS: Unemployed people (Unemployed is defined as someone that is not currently working at a full-time job, AND is collecting unemployment from a state agency). Proof of unemployment via a recent dated form from the state (but form can be no older than 30 days from registration date) with your name on it MUST be provided at time of registration, in order to receive this discount.
NOTE: Unemployed discounts must be pre-approved via a request to registration@newenglandsupplychainconference.org, and cannot be entered via the on-line registration process. To get approved:
A) Please PRINT (first) and manually fill out the mail-in form (with no payment/credit card information yet),
and
B) Scan and send in the filled-in form as an e-mail attachment, when requesting your unemployed discount (along with a scanned copy of your state agency's proof of unemployment, as specified above).
C) Upon approval, you will then be instructed where to mail in the pre-approved form with the discounted payment.

METHOD OF PAYMENT:
Payment must accompany the registration form. Secure credit card processing (preferred method) and mail-in registration is available via the links above.  The Non-member rate will be applied to registrations that do not include a sponsoring organization membership number.

CANCELLATION POLICY:
Requests for substitution or cancellations must be received by Friday, September 16, 2016 via email to eryan@instituteforsupplymanagement.org.Telephone substitutions or cancellations will not be accepted. A $100 processing fee will be assessed on all approved cancellations received by Friday, September 16, 2016. Approved refunds will be processed 15 business days after the written request is received. After Friday, September 16, 2016, no refunds will be issued. Refunds will not be given for failure to attend, late arrival, unattended events, or early departure.

SPECIAL REQUESTS:
If you have a disability or dietary specifications that require special services, please mark the appropriate box on the registration form and attach a written description of your needs.

PHOTO RELEASE:
From time to time, we use photographs and video of the conference and its participants in our promotional material. By virtue of your attendance, you agree to the use of your likeness in such material.
